I UNDERSTAND THAT ATTENDANCE OF A DOG OBEDIENCE CLASS IS NOT WITHOUT RISK TO MYSELF, MEMBERS OF MY FAMILY OR GUESTS WHO MAY ATTEND, OR MY DOG, BECAUSE SOME OF THE DOGS TO WHICH I WILL BE EXPOSED MAY BE DIFFICULT TO CONTROL AND MAY BE THE CAUSE OF INJURY EVEN WHEN HANDLED WITH THE GREATEST AMOUNT OF CARE I HEREBY WAIVE AND RELEASE BIG TAIL WAGGIN' ITS EMPLOYEES, OFFICERS, MEMBERS AND AGENTS FROM ANY AND ALL LIABILITY OF ANY NATURE FOR INJURY OR DAMAGE WHICH I OR MY DOG MAY SUFFER, INCLUDING SPECIFICALLY BUT WITHOUT LIMITATION ANY INJURY OR DAMAGE RESULTING FROM THE ACTION OF ANY DOG AND I EXPRESSLY ASSUME THE RISK OF SUCH DAMAGE OR INJURY WHILE ATTENDING ANY TRAINING SESSION OR ANY OTHER FUNCTION OF BIG TAIL WAGGIN' OR WHILE ON THE TRAINING GROUNDS OR IN ANY AREA SURROUNDING. Enrollment Contract

We are delighted that you have chosen to train your dog with us. PLEASE NOTE that tuition is NOT REFUNDABLE once submitted. Your application will be processed upon receipt of your tuition. A check mark in the box below indicates agreement to terms. Without such an agreement our generous make-up policy and the strictly limited class size (enrollment) would not be possible.

Each Companion Dog Training Program Includes...
1. six one hour training sessions 2. complimentary email consults, these are to be limited to questions about class room exercises and solutions for mild behavior problems excluding aggression 3. make-ups for classes missed (LIMIT 2 PER COURSE), these must be redeemed by attending the appropriate training level, one-to-one make-up sessions can be scheduled for an additional fee 4. homework sheets and handouts 5. a graduation certificate 6. a squeaky, fluffy toy 7. an Olympic style "gold" medal 8. a friendly, helpful instructor and an assistant. Small class size ensures premium quality instruction and personal attention for each and every dog and handler team.
